١. إن كنت تستطيع تسجيل الدخول

أسرع طريقة لحذف حسابك هي من داخل التطبيق مباشرة — عضو الطاقم من الإعدادات، والمريض من بوابته. الحذف هناك فوري ومُتحقَّق منه تلقائيًا (أنت مسجَّل الدخول بالفعل)، ولا حاجة لهذا النموذج.

٢. إن كنت لا تستطيع الدخول

استخدم النموذج أدناه. بما أنّ هذا الطلب يصل من زائر غير مُتحقَّق من هويته، سيتواصل معك فريق العيادة أولًا للتأكد من هويتك قبل تنفيذ أي إجراء على أي حساب.

٣. ما الذي يحدث بعد أي طلب حذف

• فورًا: يُعطَّل الحساب ويُمنَع الدخول به.
• خلال ٣٠ يومًا: تُزال هوية الدخول (حساب الدخول) وأي رموز إشعارات على الأجهزة.
• السجلّ السريري (الزيارات، الصور، الوصفات) لا يُحذَف فورًا أبدًا — تحتفظ به العيادة المسؤولة للمدّة التي يقتضيها القانون (خمس سنوات افتراضيًا، أو أكثر إذا ألزم القانون)، ثم يخضع للمراجعة والإزالة.
• تُخطَر العيادة المعنية بكل طلب حذف يصلها.

٤. طلب الحذف (لمن لا يستطيع الدخول)

املأ النموذج أدناه — اسمك وطريقة تواصل (هاتف أو بريد إلكتروني) وسبب اختياري. لن يُنشأ أو يُحذف أي حساب تلقائيًا من هذا النموذج؛ هو طلب يصل إلى العيادة للمتابعة البشرية.

1. If you can sign in

The fastest way to delete your account is from inside the app itself — a staff member from Settings, a patient from their portal. Deletion there is instant and automatically verified (you are already signed in) — you do not need this form.

2. If you cannot sign in

Use the form below. Since this request arrives from an unverified visitor, the clinic will contact you first to confirm your identity before taking any action on any account.

3. What happens after any deletion request

• Immediately: the account is deactivated and sign-in is blocked.
• Within 30 days: the sign-in identity (login account) and any device notification tokens are removed.
• Clinical records (visits, photos, prescriptions) are never deleted on the spot — the responsible clinic retains them for as long as law requires (five years by default, or longer where the law demands), after which they are reviewed for removal.
• The relevant clinic is notified of every deletion request it receives.

4. Request deletion (for those who cannot sign in)
